At VonOstir Innovation Services, PLLC we have partnered with Aspire Integrated Healthcare Solutions to offer patients our Virtual Care Navigator program. Our tools helps providers and care teams track and monitor the client's mental health status outside of in-person or live telehealth provider visits. You can easily respond on your phone or computer to their clinical questions. It then scores your status so that our care team can track your progress and intervene.
Waiting for Therapy
Sometimes it can be slow to get an appointment with a mental health provider. We have tools and our Virtual Care Monitoring tool to help you during this waiting period.
Hopital Post-Discharge
The Aspire Hospital Post-Discharge Follow-up program is for hospitals, providers, individuals and other organizations an additional follow-up program with resources, outpatient services, and technology tools to help improve the mental health status of patients after they leave a hospital or facility setting.
