At VonOstir Innovation Services, P.L.L.C., we require all new clients to complete our intake paperwork packet virtually (provided through our portal) or in person (paper). Please note we will not schedule an appointment until all paperwork is completed. At the time of your first visit, please bring your insurance card, a form of payment, and a state-issued I.D. for a member of our staff to take a copy of your information for the record. 

Good Faith Estimate

You or your child (as the client) have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the Center for Medicare and Medicaid, healthcare providers must give patients who don't have insurance or are not using insurance an estimate of the bill for medical items and services. 

• You or your child (as the client)  have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes costs like medical tests, prescription drugs, equipment, and hospital fees. 

• Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You or your child (as the client)  can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you or your child (as the client)  schedule an item or service. 

• If you receive a bill of at least $400 more than your Good Faith Estimate, you can dispute the bill. 

• Make sure to save a copy or picture of your Good Faith Estimate.

The cost of sessions is as follows:

Licensed Mental Health Provider 

· 90791- Initial Evaluation: $250.00

· 90832- 30-minute Individual session- $125.00

· 90834/90846- 45-minute Individual session and family therapy: $150.00

· 90837/90847- 60-minute individual session and family therapy: $200.00

· X0001- 45 minutes Life Coaching-$75.00

· Assessment Evaluation and Diagnostic testing- $2500.00 for ADHD, Autism, or OCD Screenings

·  Functional Behavioral Assessment/Behavioral Intervention Plans- $250.00

* Insurance Billing is available. We accept Aetna, Anthem, Blue Cross Blue Shield (Florida Only), Cigna, (Medicare), and Optum/United Healthcare. 

** Some Licensed Providers may utilize the +90785 Interactive Complexity CPT Code at $30.00 per session. 

Graduate Student Intern

· 90791- Initial Evaluation: $ Rate Varies (Conducted in conjunction with a  licensed provider)

· 90834/90846- 45-minute Individual session and family therapy: $30.00

· 90837/90847- 60-minute individual session and family therapy: $40.00

The fee for each session will be due at the time or after the session. Cash, Visa, MasterCard, Care Credit, or Checks are accepted for Payment. For ease of billing, you may provide your credit card information in advance and may choose to put your card encrypted on file through our Square credit card processing system. If this is the case, You or your child (as the client) understand you or your child (as the client)  must sign a payment authorization form. 

Insurance companies have many rules and requirements specific to certain plans. Your responsibility is to determine your insurance company's policies, co-pay, and deductible. A receipt of Payment may be used as a statement for insurance if applicable to you, VonOstir Innovation Services, P.L.L.C. can generate a Superbill for you. Please note that we will bill your health insurance on your behalf and accept Payment from your insurance carrier.

If your insurance does not cover mental health services or you do not have health insurance, please consider visiting the Open Path Collective. We are proud to partner with you and are pleased to honor their rates.

Keeping you, the client, in mind, our fee-for-service private payment rates are designed to work within your budget while allowing us to provide high-quality treatment. To discuss our mental health counseling rates, life coaching, or consulting, please schedule a time to speak with Mr. Oster. 

Currently, we accept Paypal, Visa, Mastercard, Stripe, Health saving Accounts, Venmo.

VonOstir Innovation Services, P.L.L.C.,  as a mental healthcare company, take you (the client) and our staff's health seriously. At this time, we have begun to phase back into seeing clients face-to-face, but please note our company follows all cleaning recommendations, social distancing, and monitoring of COVID-19 as prescribed by the C.D.C.   

As a mental health counseling, life coaching, and consulting practice, we at VonOstir Innovation Services, P.L.L.C., provide services in-office, in-home, in-school, or telehealth. At this time, Mr. Oster, L.B.S., L.I.C.S.W, L.C.S.W.-Q.S., is the only team member to provide in-home and in-community-based services. All consulting services are conducted in-office or virtually. 

VonOstir Innovation Services, P.L.L.C., are open Mondays, Tuesdays- Thursdays, and Fridays from 8 AM to 8 PM, Saturdays from 8 AM to 12 PM, and we are closed Wednesdays and Sundays.  

Our hours may change based on the needs of the company. In addition, we offer extended hours to accommodate the needs of children, adolescents, and young adults during the academic school year. 

We are closed on all local, state, and federal holidays. This includes Memorial Day, Labor Day, Thanksgiving, and the day after, July 4th, December 24th – 25th, & December 31st– January 1st.

With November 2023 around the corner, VonOstir Innovation Services, L.L.C.,  will be closed for the Thanksgiving Holiday on November 25th, 2022. 

Throughout the year, there may be times when our office closes as a member(s) of our team may provide disaster response and recovery to the whole community, thus requiring the office’s closure. If this occurs, we will post on our social media outlets, website and update our voicemail to reflect our closure.  

If you seek a Mental Health, Life Coaching, or Consulting Appointment, please call our office with inquiries and requests. If we cannot answer the phone due to being in session or away from our desk, please leave a message, and someone from our friendly and helpful team will be in touch within 24 hours. 

If you are new to the practice, we are excited to speak with you about our services. When you leave a message, please let us know what service(s) you are interested in to get the appropriate new client paperwork. All mental health and life coaching appointments are 45-to 60 minutes in length.
