 At VonOstir Innovation Services, P.L.L.C., we require all new clients to complete our intake paperwork packet either virtually (through our portal) or in person (on paper). Please note we will not schedule an appointment until all paperwork is completed. At the time of your first visit, please bring your insurance card, a form of payment, and a state-issued ID, which a member of our staff will use to take a copy of your information for our records.  

Good Faith Estimate

You or your child (as the client) has the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the Centers for Medicare & Medicaid Services, healthcare providers must provide patients who don't have insurance or aren't using insurance with an estimate of the cost of medical items and services. 

• You or your child (as the client)  have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes costs like medical tests, prescription drugs, equipment, and hospital fees. 

• Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You or your child (as the client)  can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you or your child (as the client)  schedule an item or service. 

• If you receive a bill of at least $400 more than your Good Faith Estimate, you can dispute the bill. 

• Make sure to save a copy or picture of your Good Faith Estimate.

The cost of sessions is as follows:

Licensed Mental Health Provider 

· 90791- Initial Evaluation: $300.00

· 90832- 30-minute Individual session- $125.00

· 90834/90846- 45-minute Individual session and family therapy: $200.00

· 90837/90847- 60-minute individual session and family therapy: $250.00

· Assessment, Evaluation, and Diagnostic testing- prices vary

·  Functional Behavioral Assessment/Behavioral Intervention Plans- $250.00

* Insurance Billing is available. 

** Some Licensed Providers may utilize the +90785 Interactive Complexity CPT Code at $50.00 per session. 

Graduate Student Intern

· 90791- Initial Evaluation: $ Rate Varies (Conducted in conjunction with a  licensed provider)

· 90834/90846- 45-minute Individual session and family therapy: $40.00

· 90837/90847- 60-minute individual session and family therapy: $50.00

The fee for each session will be due at the timeof or after the session. Cash, Visa, MasterCard, Care Credit, or Checks are accepted for Payment. For ease of billing, you may provide your credit card information in advance and may choose to put your card on file through our Square credit card processing system. If this is the case, You or your child (as the client) understand you or your child (as the client)  must sign a payment authorization form. 

Insurance companies have many rules and requirements specific to certain plans. Your responsibility is to determine your insurance company's policies, co-pay, and deductible. A receipt of Payment may be used as a statement for insurance,e if applicable. VonOstir Innovation Services, P.L.L.C. can generate a Superbill for you. Please note that we will bill your health insurance on your behalf and accept Payment from your insurance carrier.

At this time, our providers in Florida and Minnesota can bill insurance on behalf of our clients for assessment, evaluation, and testing services. Our current insurances in PA and VA does not cover assessment services. 

At the heart of innovation is our commitment to the needs of the community and clients. Therefore, we commit to keeping our prices affordable as a mental health practice. If your insurance does not cover mental health services or you do not have health insurance, please consider visiting the Open Path Collective. We are proud to partner with you and are pleased to honor your rates.

Keeping you, the client, in mind, our fee-for-service private payment rates are designed to work within your budget while allowing us to provide high-quality treatment. To discuss our mental health counseling rates, life coaching, or consulting, please schedule a time to speak with Mr. Oster. 

Currently, we accept CareCredit, PayPal, Visa, Mastercard, Stripe, Health Savings Accounts, and Venmo.

 VonOstir Innovation Services, P.L.L.C. is open Monday through Friday from 8 AM to 8 PM, depending on the provider, and closed on weekends. 

Our hours may change according to the company's needs. Additionally, we offer extended hours to accommodate the needs of children, adolescents, and young adults during the academic year. 

We are closed on all local, state, and federal holidays. This includes Memorial Day, Labor Day, Thanksgiving, and the day after, July 4th, December 24th – 25th, & December 31st– January 1st.

If we cannot answer the phone due to being in a session or away from our desk, please leave a message. A member of our friendly and helpful team will be in touch within 24 hours. 

If you are new to our practice, we are excited to discuss our services with you. When you leave a message, please let us know which service(s) you are interested in, so we can provide you with the appropriate new client paperwork. All mental health and life coaching appointments are 45-60 minutes in length.
